Duties and Responsibilities
- Manages FCC Team Leads by providing coaching, development, and support to build effective and engaged field teams
- Monitors team performance, addresses resource needs, and ensures consistent execution of care coordination workflows and protocols
- Analyzes and presents team performance data, productivity metrics, and quality indicators to departmental leadership
- Collaborates with Team Leads to address and resolve member issues, ensuring a seamless and compassionate care experience
- Serves as a key leader in implementing new systems, workflows, and process changes while supporting team adaptability
- Supports hiring, onboarding, performance reviews, and ongoing professional development for FCC team members through their Team Leads
- Identifies operational bottlenecks and leads efforts to improve efficiency and service delivery
- Ensures adherence to HIPAA standards, regulatory guidelines, and documentation requirements
- Additional duties as assigned
Minimum Qualifications:
- Bachelor’s degree from an accredited institution or equivalent work experience
- 3 years of supervisory experience in healthcare operations, human services, or a managed care setting, preferably overseeing field-based operations
- Demonstrated professionalism with prior leadership experience along with the ability to develop, direct, and support a team
- Demonstrated ability to build and foster effective relationships
- Experience in a role requiring independent-critical thinking, time management, problem solving and achieving goals
- Ability to travel to any HF offices for meetings and/or trainings
- Excellent verbal and written communication skills
- Proficient computer skills experience, including proficiency with Microsoft Outlook, Word and Excel
Preferred Qualifications:
- 5+ years of experience in healthcare operations, care management, or a related field
- Previous experience in home care services
- Previous experience leading a field-based operations team
- Bilingual preferred (especially Spanish, Mandarin, Cantonese)
- Skilled in interpreting data to inform strategy and drive performance
- Familiarity with medical terminology and healthcare compliance standards
Hiring Range*:
Greater New York City Area (NY, NJ, CT residents): $100,900 - $145,775
All Other Locations (within approved locations): $86,500 - $128,690
As a candidate for this position, your salary and related elements of compensation will be contingent upon your work experience, education, licenses and certifications, and any other factors Healthfirst deems pertinent to the hiring decision.
