Employment and Recruiting Privacy Statement for HF Management Services, LLC


This Employment and Recruiting Privacy Statement applies to any employees, contractors, job applicants, and prospective employees of HF Management Services, LLC (“Healthfirst”, “we,” “us,” or “our”) about whom we have collected personal information in the course of your employment or application for employment with us (collectively, “you”).This Privacy Statement pertains to your information if you have communicated with us regarding potential employment, applied for employment with us, or are currently employed by us.

  • Financial information, such as bank account number, routing number, credit or debit card numbers, and other information required to administer payroll, reimburse business expenses, and conduct general business transactions such on as the Healthfirst merchandise store;
  • Information relating to Internet and network activity, such as information relating to your web browsing sessions including navigation to and from websites and information regarding your device, operating system, and/or browser;
  • Audio, electronic, or visual information, such as audio recordings of customer service calls; video surveillance information; and facilities physical access records;
  • Demographic and other sensitive information, such as your age and/or date of birth; gender; nationality; racial or ethnic origin; marital status; criminal history; drug test results; and trade union membership.
  • Medical and health information, such as your health insurance information or workers’ compensation claim information.
